Reporting to the Clinical Health Program Manager the Nurse Practitioner provides service to program participants (experiencing intellectual challenge and/or mental illness) who access clinical health services. The Nurse works closely with the Clinical Health Program Manager and Administrative Assistant to observe, plan, and implement person-centered clinical health programming aimed at maximizing participant wellbeing and overcoming barriers to quality medical care.

The Nurse Practitioner additionally provides medical education and training to participants and staff. The Nurse is relied upon to provide leadership and contribute to program maintenance and development.

Turning Leaf Intensive Home Supports are designed to provide 24-hour, in facility, person-centered supports to intellectually challenged adults and those living with mental illness. Service Coordinators are utilized in shift staffed locations that exceed regular shift staffed home environments/criteria (e.g. more than 4 individuals), or where the individuals have significant systems and clinical requirements that require additional skills/specialization (ex. Complex dual diagnoses, significant behavioral or psychological and psychiatric intervention) and exceed the traditional support model and therefore Supervisor role – as determined by the COO.

Service Coordinator are primarily responsible for two main areas of oversight within their assigned location (home). The first, overseeing the daily operation of the home, including but not limited to provision of services to individuals as outlined in the support plan, ongoing maintenance of the home and adherence to licensing and provincial standards, financial management, support development and supervision of the team of support staff. The second, engaging in service coordination and case management related tasks including contributions to Support Planning, Behavioral Planning/Intervention, and the development of the overall treatment plan for individuals.

A Direct Support Professional will provide our service to participants who require support in the community and day program settings.  Direct Support Professionals work closely with Team lead/Supervisor to observe, plan, and implement activities to facilitate learning and development in daily living, social and life skills specific to the participant’s individual needs.

Community support is supporting adult individuals in the community, where much of the focus is on building life/social skills and vocational opportunities. Through ongoing support, individuals will become familiar with recourses available within their community, receiving assistance with advocacy around medical, legal, and housing matters.

Day Program support offers support to adult individuals in program-based community setting. Days are structured around building life skills, vocational opportunities while adding a social element with like-minded peers. The balance of skill-based learning combined with creative activities around the community allows for those who attend to gain peer connections as will as access to resources within their community.

The Pas Day Program Supervisor

Reporting to the Director of Supported Independent Living and Senior Community Clinical Case Managers’, the Community Clinical Case Supervisor is responsible for the operations of all services provided to all Turning Leaf participants within their assigned region.

Specifically, the Community Clinical Case Supervisor is responsible for overseeing the treatment/support service, management, and ensuring that program objectives and individual support plans/contracts are delivered and developed and maintained according to organizational standards.

Turning Leaf (Inc.) Community Support Service are designed to provide person-centered supports to individuals living with intellectual challenges or mental illness. The Program Coordinator (Saskatchewan) will work closely with the Director/ COO of IHS and CBT to aid in the oversight of both programs for all regions in Saskatchewan. The Program Coordinator will perform a variety of administrative and supervisory tasks, while providing support to the leadership and direct support teams. 

The Position is responsible for ensuring that services in this region are aligned with the overall Agency Mission and Values and are in alignment with the services delivered in all locations (for consistency) except where required by Legislation. The Program Coordinator will be responsible for the oversight of quality assurance of participant service, there will be no involvement in the day-to-day operations of the files. The Program Coordinator (Saskatchewan) will liaise with multiple governing bodies and be responsible for relationship building and networking within the province. Maintaining a close connection to counterparts in other Turning Leaf branches (Manitoba) will be a priority, as well as navigating services in multiple branches within Saskatchewan.

Turning Leaf Community Support Services Inc. Guided Living Services are designed to provide one-on-one, person-centered residential services by pairing participants with families in the community, assisting the participant in fostering life skills, healthy decision making, as well as providing them with a sense of family and access to that family’s community.

The successful candidate will be responsible for opening up their home and life to the individual receiving supports. Providing supervision, guidance, and companionship as well as a safe and welcoming home that offers the opportunity for inclusion.
